Award for Outstanding Educational Collaboration
This award is made in honor of Richard Gorlin, MD, world-renowned cardiologist, educator, clinician and researcher.
Purpose:
To recognize those organizations best demonstrating innovation or uniqueness in achieving effective CME collaboration.
Nomination Criteria:
To be considered for this award,

The nomination must have been submitted by a member of the Alliance.

• |
At least one of the primary individuals involved in the planning, development and implementation of the activity must be a member of the Alliance and listed as a nominee.
|
|
The activity must have been completed between January 1, 2008 and July 1, 2009.
|
• |
The nomination does not have to represent a certified CME activity but should be related to the CME process.
|
• |
A completed nomination application must include identification of related ACCME Criteria and Alliance Competency and completed Publication Release Form.
|
|
The activity/project must not have been nominated for another Alliance award.
|
|
The nominee and recipients cannot be a member of the Award Selection Panel for this award.
|
|
Only one application per nominee (person or organization) may be submitted per award category.
|
|
A completed nomination application must have been received at the Alliance office by the submission deadline. |
Evaluative Criteria:
Evaluation of the nominees for this award includes consideration of,

Nature of the CME collaboration. |
• |
Innovation and uniqueness of the activity or process. |
|
Significant contribution to CME. |
• |
Impact of this collaboration (number of people impacted, data on impact of activity, how healthcare was impacted and/or the potential for future impact) |
Application:
The Award Selection Panel will accept and review nominations received by the deadline of Wednesday, July 1, 2009. A letter of nomination should include the following:

No more than four (4) typed (not handwritten) pages. The total page count does not include the face pages.

Pages must be formatted with 1” margins on all sides and no smaller than Arial 11pt type.

Description of the contributions of the organizations in detail and explanation of why the organizations being nominated warrant recognition, including the nature of the collaboration, identification of the collaborating partners and responsibilities of each partner, and explanation of the significance of the contribution to CME.

Link the submission to Nomination and Evaluative Criteria, and include identification of ACCME Criteria and Alliance Competency, and completed Publication Release Form.

Verification nominees represent the organization referenced on the application (signed attestation on organization’s letterhead).

Do not include additional activity-related supplemental materials. These materials will not be reviewed.
|

Applications may include up to a two (2) page Outcomes Appendix, if applicable, detailing impact/outcomes data.

|
Submission of six (6) copies for receipt at the Alliance by Wednesday, July 1, 2009 (submitted materials will not be returned). |
|
Only one application per nominee (person or organization) will be accepted for review. |
Recognition:
One (1) complimentary registration for the 2010 Alliance Annual Meeting; the opportunity to showcase the activity by exhibiting a poster during the Annual Conference; a “spotlight” article in the Alliance’s Almanac newsletter; and a plaque commemorating the award presented at the 2010 Annual Conference.
