Attendance The Alliance for Continuing Medical Education designates this educational activity for a maximum of 21.5 hours. You should only claim hours commensurate with your participation in the activity. The reporting of attendance hours is done on an honor system and an hour for hour participation basis. The hours claimed should not exceed 7 hours on Wednesday, 4.5 hours on Thursday, 7 hours on Friday, and 3 hours on Saturday, with a total maximum hours of 21.5. Instructions To receive a certificate of attendance, complete this form; make certain to provide your e-mail address, and submit it for receipt prior to 5:00 pm CST, Friday, February 26, 2010.
Attendance Certificate An attendance certificate will be sent by e-mail to you after receipt of this completed form. Please save the Final Program for your records, should additional information be requested from you.
